Lincoln Township Public Library is a Passport Acceptance Facility. We provide the same level of service as other Passport Acceptance Facilities for all U.S. Citizens at a convenient location for our community. Click here to download the LTPL Passport Services brochure.
You must apply in person if ANY of the following are true:
- You have never been issued a U.S. passport
- You were under age 16 when your previous passport was issued
- Your most recent U.S. passport was issued more than 15 years ago
- Your most recent U.S. passport was lost or stolen
- Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change
How To Apply
Step 1: Complete DS-11 Form
Complete all parts of the form. Do not sign your form until asked to do so by a passport acceptance agent or employee.
Ways to complete your form:
- Enter the information online and print form DS-11
- Print a blank form DS-11 and complete by hand. If you’re completing the DS-11 at the library or at home, do not sign the form until you are with the Passport Agent.
Note: if printing, print single sided and bring all pages including instructions with you to the appointment. If filling out by hand, please print legibly and use black ink. DO NOT sign the DS-11 form until the Passport Agent instructs you to do so.
Step 2: Evidence of U.S. Citizenship
Your evidence must be an original or replacement copy. The document must have the official seal or stamp of the office which issued it. You must submit one of the following documents:

- U.S. birth certificate that meets the following requirements required by the U.S. Dept. of State
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship
- Full validity, undamaged U.S. passport
If you cannot submit one of these documents, go to the Citizenship Evidence page for more information. Both a photocopy and original document will be sent to obtain your passport. Photocopies can be made at the library for an additional charge.
Step 3: Photo ID
Proof of Identity (If applicable, bring along one b/w or color copy of the secondary ID). Photocopies can be made at the library for an additional charge.

- Valid Michigan driver’s license or state ID.
- If your ID was issued less than a year ago, you MUST bring a Department of State Approved Secondary Identification (Previously issued ID even if it is expired is acceptable)
- For non-Michigan residents, you MUST bring your driver’s license or State ID and a Department of State Approved Secondary Identification

Step 4: Photo
You must provide one photo with your application. Go to our Passport Photo page for photo requirements and to see examples of photos. The library does not do photos. Photos can be done at our local Walgreens or CVS stores.
- Submit one color photo
- Submit a recent photo taken in last 6 months
- Use a clear image of your face
Step 4: Fees
When applying, you will pay two separate fees – an application fee and an execution (acceptance) fee. You will pay the application fee to the U.S. Department of State, and the execution (acceptance) fee to Lincoln Township Public Library which takes your application.

- Application fee: payable to the U.S. Department of State by check or money order ONLY. See the fees section below for a listing of fees.
- Acceptance fee: A separate payment of $35 per application is required for any passport facility. Payable to Lincoln Township Public Library by check, money order, or credit card (VISA, Mastercard, or Discover). Copies for citizenship evidence and IDs is not included in this fee. Optional $21.36 USPS express mail (1-2 day delivery) fee for sending the passport application to the U.S. Department of State (express mail service only available Monday through Friday).
- If applicable, additional fees will be made for expedited service or other special services.

Service is by appointment only and are subject to availability. We are available by appointment to process applications during the following hours at the Library.

If you need your passport quickly (i.e. life or death emergency, traveling within two weeks), you can expedite your application. Please visit travel.state.gov’s “Get a Passport in a Hurry” page prior to calling the library.
Applicants (Age 16 &17)
- For 16 and 17 year olds, parental awareness must be shown. Parental awareness can be shown by a parent appearing with the applicant in person or a signed statement consenting to the issuance of a passport from one of the applicants parents or legal guardians.

Minor Applicants (Under Age 16)
- Both parents or guardians must approve that we can issue a passport to a child, and go with the child to apply in person.
- If one or both parents or guardians cannot apply with their child, you will need to show us more documents.
- You cannot renew your child’s passport using Form DS-82.
- Passports for children under age 16 are only valid for 5 years.

Renewals CANNOT be handled by the Library although someone can help you fill out your form if you need help. If you have a passport that was issued less than 15 years ago and you were at least 16 years old when it was issued, a self-service renewal by mail is likely the correct option. Visit travel.state.gov’s “Passport Renewal” page to confirm that renewal by mail is appropriate.
Note: If your last passport was issued when you were under 16, you will need to fill out a new DS-11 form. The Library accepts appointments for this form.
LTPL does not do passport photos. Please visit a local Walgreens or CVS for passport photos before you visit the Library. Photo tips:
- Submit one color photo
- Submit a recent photo taken in last 6 months
- Use a clear image of your face
- Do not change your photo using computer software, phone apps or filters, or artificial intelligence
- Have someone else take your photo. No selfies.
- Take off your eyeglasses for your photo
- Use a white or off-white background without shadows, texture, or lines

Once your application has been completed by a Passport Acceptance Agent at the Library, all questions MUST be directed to the National Passport Information Center. We are not responsible for the processing time of your passport.
